Greetings again from the darkness. Why is it that estranged mothers always seem to show up when we are frantically trying to clean up all evidence of a murder that took place in our apartment? OK, maybe that's not really a common occurrence, but it's certainly at the heart of this Hitchcockian psychological mind-bender from writer/director Rupert Jones. His brother, the very talented and always interesting Toby Jones, stars as the quiet ex-con attempting to get his life on track. A pre-credit opening scene has Carl (Toby Jones) borrowing an uncharacteristically flashy (and quite hideous) shirt from a helpful neighbor for his date which was arranged online. After passing out on the sofa, Carl discovers his date Abby (Sinead Matthews) dead in the bathroom and flashes back to a brief moment of violence. Much of our effort goes into slowly assembling the pieces and clues that are doled out along the way, and it takes a sharp eye to catch some of them while we are challenged by others to determine if they are dreams, or actual memories. I nfluenza viruses are among the most unpredictable disease actors around. These constantly changing germs regularly humiliate anyone who is rash enough to forecast the potential severity of an upcoming flu season or how well — or poorly — the might work this year. “I wouldn’t,” Dr. Kanta Subbarao, director of the World Health Organization’s influenza collaborating center in Australia, said with a laugh when asked what she would project the Northern Hemisphere might be facing, flu-wise, in the coming months. “I’ve been in this business too long to fall into the trap of trying to predict.” That said, flu experts are a bit worried right now. There are some signals they think may foretell that we’re facing a nasty flu season. But they hand-to-heart don’t know whether the constellation of things that is worrying them will lead to clogged doctors offices. STAT talked to Subbarao and other flu experts about the coming flu season. Beyond the basics — there will be a wave or two of flu activity sometime between now and next spring, and the people caught up in the wave will curse the virus — we cannot tell you what’s going to happen. But we can explain why experts are on edge. Here are three things they’ve been watching. The Southern Hemisphere had a nasty flu season In the flu world, scientists in one hemisphere keep a close eye on the other to try to figure out what might be headed their way. And during their 2017 winter, parts of the Southern Hemisphere, including Australia, had a pretty nasty flu season. Hong Kong and some other places in the tropics have also experienced a of flu of late. The illness has been caused mainly by H3N2 viruses, which trigger the worst outbreaks of the two influenza A viruses and two types of influenza B viruses that circulate among people. Flu seasons in which H3N2 viruses dominate are typically miserable, with lots of hospitalizations and more deaths. “At this point the data look like it was a big season. Started earlier than usual, lasted a bit longer. And all segments of the population were affected, including people who were vaccinated,” Subbarao said of the Australian winter outbreak. The severity of the flu in at least some parts of the Southern Hemisphere has created concern a similar assault is heading toward the Northern Hemisphere. But there’s a complicating factor: Last winter North America had a pretty active flu season, caused mainly by H3N2 viruses. So did countries in the Southern Hemisphere follow us? Or are we about to follow them? “That’s the million-dollar question,’’ Sabbarao said. People who were infected by the virus last year are less likely to fall prey to it again this winter. So the amount of illness experienced in North America last year could blunt the impact of this season, if another H3N2 onslaught is headed our way, suggested Jacqueline Katz, deputy director of the influenza division at the Centers for Disease Control and Prevention. But we could be facing a season in which H1N1 viruses — which tend to cause less severe outbreaks — dominate. Or we could have what’s known as a mixed season, with all the flu viruses that infect people taking a substantial share of the pie. There have been few mixed flu seasons in recent years, noted Dr. Daniel Jernigan, director of the CDC’s influenza division, who added — perhaps hopefully — “It could be if that trend follows that maybe we’ll have an H1N1 year.” What’s the likelihood we would suffer through back-to-back H3N2 seasons? Danuta Skowronski, a flu expert with British Columbia’s Center for Disease Control, said that does happen on occasion. And the second or third wave of H3N2 can actually be worse than the season or seasons that proceeded it. “The bottom line is you should never say never with influenza,” Skowronski said. The flu vaccine is probably not an optimal match Late last month leading flu experts from around the world — Katz and Subbarao among them — met at the World Health Organization for a twice-annual ritual. They selected the viruses that should be covered by the winter 2018 Southern Hemisphere flu vaccine, using recent viral surveillance data from flu laboratories around the globe. It takes months to make and distribute flu vaccine, which is updated regularly in an effort to keep pace with the ever-changing viruses. The strain selection meeting for the Northern Hemisphere vaccine takes place every year in late February. Rather than recommend a flu shot with the same composition as the ones clinics, pharmacies, and doctors’ offices in the Northern Hemisphere are injecting right now, the experts decided to update two of the target viruses. Interpretation: The Northern Hemisphere shot is already a bit out of date. That happens more often than experts would like. Sometimes the differences between the viruses in the shot and the circulating viruses are significant; in those years, lots of people who got vaccinated get sick anyway, and there are lots of headlines about flu shot “mismatches.” The H3N2 component of the vaccine was one of the ones that was updated. In light of the damage that family of viruses does, that’s not a terrific sign. But Katz said the update was “incremental” rather than a big switch, so perhaps that component of the vaccine won’t be way off target. It’s worth remembering, however, that the H3N2 component we’re getting now was in the winter 2017 Southern Hemisphere flu shot. And as Subbarao noted, some of the people who got sick in Australia had been vaccinated. If the H3N2 viruses that circulated in Australia head to North America, that component of the vaccine will offer some protection “but it won’t be optimal,” she said. And Canada estimated the H3N2 component of the vaccine reduced infection risk by 34 percent and 37 percent, respectively. Given that these viruses have continued to evolve away from that target, Skowronski warned the H3N2 component would not be expected to perform better this year. “Don’t consider yourself invincible if you received the vaccine,” she cautioned. “Because there are signals to be concerned about in terms of the possible [H3N2] protection this season.” She suggested people who are at high risk of getting really sick if they catch the flu might consider asking their doctor in advance for a prescription for an antiviral drug like Tamiflu that they could fill quickly if they need it. That only really matters, however, if there is a lot of H3N2 activity this year. The repeat vaccination problem In recent years, Skowronski and others have been noticing a surprising phenomenon. Some winters people who were vaccinated two seasons (or more) in a row seem to be more apt to contract the flu than people who were only vaccinated in the season under study. The still-emerging theory suggests that when people are revaccinated with the same target virus or viruses, their immune system doesn’t generate as many antibodies as when it is presented with a new target. Then if the viruses they encounter are different from the viruses in the vaccine, the fact they generated fewer antibodies puts them at higher risk of infection. (You can read more about this issue.) This year’s Northern Hemisphere flu shot contains the exact same H3N2 component as last year’s. And as the updating of the Southern Hemisphere vaccine signals, the H3 viruses have mutated. So the repeat vaccination effect may be a factor this winter, Skowronski suggested, adding that public health authorities should be on the lookout. “There should be systems in place to assess that at the earliest possible opportunity,” she said. Again, though, that only matters if there’s a lot of H3N2 activity this season. “It will be an interesting year from many perspectives,” Skowronski predicted. Severe disease related to influenza virus infection is monitored by surveillance of hospitalized laboratory-confirmed influenza cases or severe acute respiratory infections (SARI) in some countries. For week 47/2017, relatively low numbers of severe cases were reported by countries operating these surveillance systems. For week 47/2017, few laboratory-confirmed influenza-infected cases from intensive care units (ICU) or other wards were reported by France (n=2), Ireland (n=2), Spain (n=7), and the United Kingdom (n=7). Since week 40/2017, 7 countries have reported laboratory-confirmed hospitalized influenza cases in ICU or other wards: 89 cases in ICU (37 in the United Kingdom, 36 in France, 11 in Spain, 3 in Sweden and 1 each in the Czech Republic and Denmark), and 61 in other wards (27 in Ireland, 18 in Denmark, 14 in Spain and 2 in the Czech Republic). Of 89 cases in ICU, 63 (71%) were infected with type A viruses (26 A(H1N1)pdm09, 14 A(H3N2), 23 A un-subtyped) and 26 (29%) with type B viruses. A similar distribution was observed in other wards: of 61 patients, 41 (67%) were infected with influenza A (10 A(H1N1)pdm09, 11 A(H3N2), 20 A un-subtyped) and 20 (33%) with influenza B viruses. Severe disease related to influenza virus infection is monitored by surveillance of hospitalized laboratory-confirmed influenza cases or severe acute respiratory infections (SARI) in some countries. For week 47/2017, 417 SARI cases were reported by 11 countries from which 197 specimens were tested with 3 (from Armenia) testing positive for influenza virus. Since week 40/2017, 5 420 SARI cases have been reported from 14 countries; of 1 560 specimens tested for influenza viruses, 13 were positive for influenza virus: 3 from Armenia (type B), 6 from Kazakhstan (1 A(H3N2) and 5 type B), 3 from Tajikistan (1 type A and 2 type B) and 1 from Ukraine (type A). Since 2015, first lady Michelle Obama has honored a school counselor of the year in a ceremony at the White House. Friday, the honor goes to Terri Tchorzynski of the Calhoun Area Career Center in Battle Creek, Mich., where she works with 11th- and 12th-graders drawn from 20 public high schools in Calhoun County. Tchorzynski started her career as a high school English teacher, before getting her master's degree in counseling — a role she says she 'always knew she wanted.' NPR Ed caught up with Tchorzynski about her work in Michigan and the important role she sees counselors playing in schools. Is it true that school counselors don't like the term 'guidance counselor' anymore? Can you explain that? We use the term 'school counselor' now. We cringe when we hear the term 'guidance counselor.' It's kind of that older-fashioned model from years ago, the, 'I'm gonna guide you towards your college or career.' It's from when people didn't really understand what was going on with the whole realm of school counseling. But now with a school counselor, we're much more than just the guidance counselor. So what is the role of a school counselor? There are three ways to break it down. First is college and career readiness, preparing our students for what life looks like after high school. If it's college, then getting them focused on FAFSA completion, college scholarships and being academically ready. If it's for a career, figuring out what pathways are appropriate for them and what careers are available to them that fit their interests and their skills. Second, we work in the social-emotional domain. This is more dealing with behavior, or with personal problems that students may have that impact their education. So we help them through those things so they can be successful in school. Third, we'll call the academic domain. This is working with attendance or grades, or making sure students have enough credits to graduate high school. So we are people that work in mental health, that work in the academic domains, that work in college and career readiness. How do you use data? Can you give me an example? We use a lot of data in our building, so that our stakeholders have a clear picture of what we're doing in the counseling department. Here's an example about modifying behavior. At the beginning of every single school year we do presentations for every single one of our students about bullying and harassment, because we know that it's an issue in our schools and it's an issue with our students and we want to make sure that it's addressed early. And then we start monitoring behavior referrals. Before we started all this, the behavior referrals were not set up a certain way. But as a counseling department, we wanted to know specifically, if there's a behavior issue that's connected to bullying or harassment, we want it marked. So we can start identifying if there are issues in certain programs — we don't have classrooms, instead we call them programs. We created this tag in our student data system, so any time a referral went through about bullying and harassment, we were flagged. We could see when and where there were incidents and tailored our interventions based on that data. We've pulled out certain groups and done some positive support groups for specific students too. What's one thing you've learned as a school counselor? A lot of times it's very easy to get centered in on the students who come to your office all the time or the students that are very outgoing. They're the A students that just want additional support. But there's a large population of students that just don't know to ask for help. Maybe they are first-time college-goers and so they don't even know what they need to know in order to go to school. So I think it's so important for counselors to think about the students that maybe don't have a voice. We need to be the voice for students that don't have the voice. It's my job as a counselor to make sure that all these students, regardless of race, ethnicity, income or what kind of backgrounds they come from, it's my job to give them the same support. So sometimes I may be more intentional about reaching out to those students because I know they won't be the ones that are coming to me first. So I guess the advice is — be proactive. We've reported about the in our nation's schools — and it's important to note how important counselors are in helping students with those types of problems. Oh, definitely. Having our students and our families understand that that's part of our role, is important. Some people just don't understand that it is a part of what we do. It's not just writing letters of recommendation and telling students where they should go to college. There's also a big part of our jobs as counselors that focuses on the social-emotional side. There is a huge need for that now. We have those community contacts, so if I have families that are uncertain about local community agencies and how they can help, part of my role is to connect them with those agencies. What has been the biggest challenge in your career so far? As a school counselor I think the social-emotional part is really hard. It's hard because students go through a lot of difficult things in their lives. And so when you're dealing with student tragedies — student death or violence — or just the things some teenagers have to face, that's really difficult. In my setting as a school counselor, there's only so much that I can do. The more we can connect with outside agencies, that helps too, so then I can refer them out to a community agency that can help them even further. But that's a difficult part of the job. It's something I don't think I'll ever get used to, because it's challenging and sad. There's a lot of things that our students have to deal with that are really difficult. Can you give me some examples of the things that have come up in the last seven years? Last year was a really difficult year for us. We had quite a few student tragedies. We actually lost one of our students to a car accident, and that obviously impacted all of us in our building. We have students that do self-harm and suicide, unfortunately. A lot of what I see nowadays with teenagers is just homelessness and poverty. I have a lot of students that are 16- or 17-years-old and for whatever reason, they just they can't live in their home, and so they need to find somewhere else to live. Sometimes, they're living with different friends, living day to day. That's always a struggle, because as a counselor, I want them to be successful, but at the same time, they're dealing with poverty and homelessness and trying to figure out where they are supposed to stay that night. So what keeps your spirits up? I just know that as a counselor, that's why I'm there. That's my role. The more students can trust me, and know that that is part of what I do and that I can help support them, the better. I always tell them, 'I don't have this magic pill that makes everything go away.' But I'm a resource for them to help them in whatever it is that they need. Unfortunately, school counselors can be rare. In Michigan, where you're from,. The American School Counselor Association recommends a ratio of 250 to 1. Clearly, that's far from the norm. What are your thoughts on that? When you think about the personnel you have in your schools, school counselors are the ones that play the role in regards to impacting academic behavior and attendance. So if you don't have a school counselor in your building, then there is a certain person that plays all of those roles and puts all those pieces together. It's so important for people to understand that counselors are needed at every single level, kindergarten through 12th grade. Elementary students have just as many needs, so it's just as important to establish early what a school counselor can do for students In Michigan, elementary school counselors are very rare. In our county I think we have one school district that has one. In my own kids' school, they have one that will float between three or four buildings. Counselors just can't offer the services that are needed for those students when they just have such large caseloads. |
